Job Title: Virtual Care Coordinator
Location: New York, NY
Duration: 13 Weeks
Shift: 3:00 PM – 11:00 PM
Pay: $27/hr
Job Summary:
"Navitas Healthcare, LLC" is seeking a Virtual Care Coordinator to support remote patient care through telehealth and monitoring platforms. This role ensures patient safety, coordinates virtual care, supports clinical teams, and helps maintain high-quality, patient-centered outcomes.
Responsibilities:
Monitor patients remotely, identify changes in condition, and escalate concerns.
Track vital signs, behavioral changes, and clinical metrics.
Support telehealth operations, including scheduling, consults, and technical assistance.
Document patient care accurately and ensure HIPAA compliance.
Participate in quality reviews, reporting, workflow optimization, and staff training.
Collaborate with clinical teams, leadership, and technology partners.
Qualifications:
Minimum 3 years of direct patient care experience.
Experience with telehealth or remote monitoring and EMR systems.
Strong computer skills, communication, and problem-solving abilities.
Preferred: Health-related degree, telehealth workflow or quality improvement experience.
Flexible to work evenings, weekends, and holidays.
